The members of the Board of Dentistry are Dr. Rannier F. Reyes, Chairman; Dr. Gloria M. Bumanlag, Dr. Steve Mark G. Gan, Dr. Melinda L. Garcia and Dr. Dominador H. Santos Jr., Members.

Registration for the issuance of Professional Identification Card (ID) and Certificate of Registration will be on January 3, 2014. Those who will register are required to bring the following: duly accomplished Oath Form or Panunumpa ng Propesyonal, current Community Tax Certificate (cedula), 1 piece passport size picture (colored with white background and complete name tag), 2 sets of metered documentary stamps and 1 short brown envelope with name and profession and to pay the Initial Registration Fee of P600 and Annual Registration Fee of P450 for 2014-2017. Successful examinees should personally register and sign in the Roster of Registered Professionals.

The Bureau of Internal Revenue announced that the submission of appointment log books and schedule of service fees is extended until May 31 2014. Dentists and other professionals are reminded of this new deadline.

Appointment log books, affidavits and other requirements are contained in BIR Order 4-2013 issued March 3, and violators face the penalty of P1,000 to P50,000 fine and one to 10 years imprisonment.

Professional Regulation Commission

The members of the Board of Dentistry who gave the licensure examination are Dr. Dominador H. Santos Jr., Chairman; Dr. Gloria M. Bumanlag, Dr. Steve Mark G. Gan, Dr. Melinda L. Garcia and Dr. Rannier F. Reyes, Members.

The results were released in two (2) working days after the last day of examination.

Registration for the issuance of Professional Identification Card (ID) and Certificate of Registration will be on December 22 & 23, 2014. Those who will register are required to bring the following: duly accomplished Oath Form or Panunumpa ng Propesyonal, current Community Tax Certificate (cedula), 1 piece passport size picture (colored with white background and complete name tag), 2 sets of metered documentary stamps and 1 short brown envelope with name and profession and to pay the Initial Registration Fee of P450 and Annual Registration Fee of P420 for 2014-2017. Successful examinees should personally register and sign in the Roster of Registered Professionals.
